How do I file a claim?

If payment for qualified services is not made using the CareFlex Express Benefits Card, participants have two options for requesting manual reimbursement. They can either submit to CareFlex by email, fax or mail a completed Request for Reimbursement form, or complete the claim reimbursement request online.

Instructions for filing a paper claim:

To submit a paper claim, participants can access a Request for Reimbursement form located under Download Forms at the top right corner of this web page. The policy holder completes the reimbursement form with necessary information; indicating the provider of service, person who incurred the claim, the charge for service, and the date of service. The completed form and appropriate supporting documentation can be mailed, faxed, or emailed to CareFlex Benefit Solutions:

To file a claim online, participants can access their account by clicking on "View My Accounts" in the bar to the left or from the homepage. For detailed instructions to create an online account, Click Here. Once you have accessed your account online, select "Request Reimbursement" under the "My Accounts" tab, and complete the necessary information. Processing Schedule:

CareFlex processes claims weekly. All claims received prior to 12 noon on Thursday will be included in that week's reimbursement process. Depending upon the selected reimbursement option by your Employer, a non-taxable reimbursement will be provided via check, direct deposit, or through payroll. Direct Deposit reimbursement can be updated through out the plan year by completing a Direct Deposit Authorization Form.
